By completing this form you are agreeing to abide by the Health and Safety Guidelines of Staffordshire University and adhere to the guidance of the member of staff leading the event/visit. You understand that if you do not arrive at the designated meeting place at the agreed time or if there is a problem with your travel then this will need to be resolved without causing a delay to the rest of the group who will continue their journey as arranged. In the event of personal delay, you will advise the visit leader on the contact number provided.
I also understand that deposits are non-refundable and that by paying the deposit you are committed to paying the full cost of the trip. The full payment must be made before the trip takes place. Any cancellations will incur cancellation charges which you will be required to pay, from the travel company that the trip is booked with.
